PERMISSION FOR USING STUDENT WORK
Please print student’s name in proper spaces.
I authorize Westbrook ISD to publish the following
information. I understand that this information may
be published on the Westbrook ISD web page, the
student’s personal web pages, in newspapers, district
or school newsletter, or used in training. I further
give Westbrook ISD permission to use my child’s work
in their publications for perpetuity.
First name, last
initial, and class will be the only identification for
my work/picture published on the Westbrook ISD web
pages, unless I indicate differently below
