Seizure Plan Form

If your child has a seizure disorder, please have his/her doctor fill out this seizure plan form from the TEA for the school nurse to have on file. This form will allow the nurse to communicate with your child's teachers and other school faculty to ensure that he or she is safe and taken care of in the best way possible.
